Substance Abuse and Addiction Assessment
As part of our Counseling Services program, Family Services offers two of the best tests available for assessing substance abuse and addiction. The Addiction Severity Index (ASI) provides an overview of the severity of the substance abuse problem. The Level of Service Inventory (LSI-R) indicates the likelihood of future abuse or relapse.
The tests can be ordered and taken separately or in combination. The results obtained give referral sources and clients the information needed to create a solid treatment plan and the guidance to set appropriate goals.
Cost for the tests is $100 each or $150 for both. Cash or money orders are required, no checks or credit cards.
Testing is by appointment only. The test will be scheduled only when full payment has been received.
